*If you are renewing your coverage, please have projected income for 2027 and your list of doctors ready.
 Names, Date of Birth, Social Security Numbers of all members in the household that will be listed on the 2026 Federal Tax Return.
 Projected Household Income for 2027 (Most recent Paystub or Last filed tax return)
 If you are a naturalized citizen, please have your Citizen Certificate document.
 If you are a Permanent Residency (Green Card), please have your Permanent Residency Card (Green Card) or any other immigration documents available.
 Correct spelling of all your doctor first and last names (Primary (PCP) and Specialists)
