If you haven’t already done so, please send me a list of your medications and doctors along with their specialty (e.g. Primary, Cardiologist etc.) Also name of preferred pharmacy. I can send you an email or text with the RxCollect to provide this information.

You’ll receive an email with a link to complete the Scope of appointment, please complete it as this is required to discuss Medicare Advantage and Prescription Drug plans. If you've already done a scope of appointment within the past 12 months a new one is not needed.
