Once you submit your appointment request, you’ll receive an email entitled, "Scope of Appointment" (SOA). Completing this form is a requirement of the Centers for Medicare & Medicaid Services and must be done before our discussion.

After I receive your completed SOA, I’ll send a confirmation email with details on what to have ready for our call.
