The Master's Choice Agency has many different major medical health plan options for individuals and families depending on their situation and needs.

If you would like to apply for or re-new a government subsidized health plan (known as an ACA plan, Obamacare, Marketplace Coverage, etc), it is important to understand that these plans use income tax credits to lower the cost of your monthly premium, deductible and co-pays. The income tax credits are based on the county where you live, your household size (as claimed on your taxes for the year of coverage), and your household's estimated taxable income (for the year you are applying for coverage). If you are applying for one of these subsidized plans, your eligibility and the level of assistance you receive is based on these factors so please be prepared to discuss them during your appointment.

Health plans that are not government subsidized do not require any income calculations. We will only need to know who needs to be incomed in the coverage.

If you decide to enroll in either type of health insurance plan you will need to provide the name, date of birth and social security number for each person included in the coverage before the application can be completed.
